When I was signing up for a library card here in Oxford, I had to fill out a small questionnaire with my name, address, etc., but I was surprised by how many choices for title pop up (regularly) for people here in the UK. I guess the old Mr. Mrs. Miss. Ms. thing just doesn't cut it here.
